The Fallbrook Healthcare Foundation (originally known as The Fallbrook Hospital Foundation) is a nonprofit organization dedicated to assessing and providing for the healthcare needs in the Fallbrook, Bonsall and DeLuz regions of North San Diego County,

Founded in 1980 as a foundation serving Fallbrook Hospital, The Foundation has grown from serving hospital needs to addressing the non-medical health needs of the community as a whole.

In recent years, The Foundation, through the generous financial support of those residing in the Fallbrook area, has implemented the following programs and services:

· Health Care Resources – An information and referral service for health resources available to all Fallbrook area residents.

· Project Care – This C ommunity A ction to R each the E lderly helps aging and disabled residents stay independent in their homes by providing taxi vouchers, minor home repairs and alliances with the Postal Service and Volunteer Police for daily home checks. “Vial of Life” containers, which list medical information and can be attached to the refrigerator, are available free of charge through this program.

· Care Van – A transportation service for medical appointments and grocery and pharmacy shopping for elderly begins March 1. Reservations for van service need to be made a week in advance.

· Elder Day Care – “The Club”, a social and recreational day care program is available beginning March 1 for those elderly residents needing care. This special program is geared to those needing a slower, calmer environment - whose eyesight may be dim, who may need care and attention to get around, and whose hearing and memory isn't as sharp as it once was. Hours are Monday and Friday from 9 to 3.

· Fallbrook Recovery and Education Center – This program run under the guidance of Mental Health Systems provides one-on-one and group counseling, training and education for anger management, substance abuse and court ordered DUI rehabilitation.

· SeniorNet – Computer classes for beginners to sophisticated build-your-own-computer and those in-between offered by dedicated and knowledgeable volunteer teachers.

· N.A.M.I. – Weekly support groups and training and education classes for family members and caregivers of those suffering from mental illness. Support groups and classes are conducted under the guidelines of the National Alliance for the Mentally Ill.

Your tax-free donation may be designated to assist any one or all of the programs of The Foundation or to assist us in bringing new programs to the community.
